Summary

Surveyors observed multiple deficiencies in the facility's physical environment, including a lack of maintenance and cleanliness in common areas and resident spaces. In the dining room, all hanging light fixtures contained dead insects, portable fans were visibly dusty and blowing air toward residents, and floor vents were coated in thick dust, debris, and cobwebs. In the kitchen, a dirty floor fan was operating and blowing air across both clean and dirty areas, including sanitized food containers.
