On multiple occasions, the resident was observed in bed with the catheter bag hanging uncovered from the bedside, including times when the bag was in full view from the doorway. The facility's policy specifically prohibits demeaning practices and requires staff to help residents keep urinary catheter bags covered. Interviews with a CNA, an LPN, and the DON confirmed that catheter bags should be placed in privacy bags, but this was not done for the resident. The resident had diagnoses including cerebral infarction, chronic pain, urinary tract infection, and benign prostatic hyperplasia, and was assessed as having intact memory and independence in daily decision making.
