Summary

The facility failed to maintain a safe, clean, and homelike environment for its residents, as evidenced by unrepaired broken bathroom sink faucets in multiple resident rooms, loose and broken door handles, and missing floor tiles in a public hallway outside the therapy room. Observations over several days confirmed that the bathroom sinks in two resident rooms had broken faucets with constant running water, and door handles in two other resident rooms were loose or broken.
