Summary

The facility failed to provide required written notifications and documentation related to resident transfers and bed-hold policies for multiple residents who were hospitalized. Specifically, for four sampled residents and one closed record, the facility did not provide the residents or their representatives with written notices of transfer or bed-hold, nor did they notify the State Long Term Care Ombudsman as required. However, documentation for the sampled residents showed missing or incomplete notices for hospitalizations, with some records lacking any notice and others missing specific required details. An administrative nurse confirmed during interviews that the required documentation was not present in the records reviewed.
