Summary
Surveyors identified that the facility failed to maintain a pest-free environment and did not have an effective pest control program in place across both resident units. Persistent fly activity was observed in multiple locations, including resident rooms, the main kitchen, staff areas, and the North Unit activity room, throughout the survey period. Specific instances included flies in the rooms of several residents, in a staff office, and small flying insects in a resident bathroom.
