Summary

The facility failed to post daily nurse staffing information in an area accessible to all residents and visitors, as required by both facility policy and regulatory requirements. Specifically, nurse staffing levels for each shift were not posted in the facility on several dates, including July 21 through July 25, 2025, and July 28 through July 29, 2025. Observations confirmed that the required information was not displayed at the reception desk, in the lobby, or on any units or hallways. During an interview, the staffing coordinator acknowledged that the information was supposed to be posted at the receptionist's desk but admitted it was not done due to lack of time while the survey team was onsite.
