In this case, a resident with dementia and dysphagia was observed by a nurse to be unable to speak, with a flushed face and watery eyes, and crumbs from a sandwich were found around the resident's chair. This incident was documented as a Facility Reported Incident. Although the investigation into the incident was completed within the required five-day period, there was no documentation showing that the Five Day Follow-Up Report was submitted to the State Survey Agency as required by policy. The DON confirmed during an interview that while the investigation was completed on time, the necessary follow-up report was not sent to the State Agency.
