This incident was also not reported to the SA within the required timeframe. Additionally, for a third resident, there was no abuse or neglect incident report present for an incident that occurred, and the administrator confirmed that this incident should have been reported to the SA as it involved allegations of neglect. Interviews with the administrator confirmed that these incidents were not reported in a timely manner as required by regulations, and the facility acknowledged the reporting failures during the survey.
