Summary

The facility failed to report allegations of abuse and neglect to the State Agency (SA) within the required two-hour timeframe for three out of four residents reviewed for grievances. In one case, a resident reported that a nursing assistant left to get supplies during care and did not return, resulting in another staff member needing to assist.
