Summary

Surveyors observed that a resident's bathroom on the C Unit was not maintained in a clean and sanitary condition. During a tour with the Unit Manager/LPN, a dry brown substance was found on the grab bars by the toilet, the back of the toilet, the wall behind the toilet, and the bathroom floor. When the Director of Housekeeping (DH) was shown these findings, he was unsure if the bathroom had been cleaned and stated he would instruct the housekeeper to clean it. However, upon a follow-up visit over two hours later, the same brown substance remained on the back of the toilet and grab bars, despite the DH stating the bathroom had been cleaned. The DH acknowledged the importance of maintaining sanitary living areas to prevent infection and noted that this particular bathroom was challenging to keep clean due to the resident's tendency to soil the area during use. The facility's policy on routine cleaning and disinfection requires detailed attention to visibly soiled surfaces and high-touch areas, including toilet seats. The observations and staff statements indicated that the required cleaning and disinfection were not adequately performed in this instance.
