Summary

The facility failed to maintain food quality and safety in accordance with professional standards, as evidenced by multiple observations of improper food storage, labeling, and handling. Surveyors found undated and unlabeled beverage cups in the refrigerator, expired and deteriorating produce, and a personal-sized bowl of food in the freezer without identification. In the pantry, overly ripe bananas with open skins and bottles of dry seasoning with open lids were observed. The Certified Dietary Manager (CDM) acknowledged the presence of expired, undated, and exposed food items and confirmed responsibility for food quality and safety. Additionally, unsanitary meal preparation practices were observed, including a staff member placing a scoop handle directly into pureed bread and repeatedly using it, and another staff member using a soapy towel from the dishwashing area to wipe a resident's plate before serving. Both staff members acknowledged these actions, and the CDM confirmed witnessing the infractions.
