During the survey, it was observed that the previous NHA's license was still posted in the hallway, despite the former administrator confirming she had not been present since May and was unaware her license remained displayed. Review of the employee list indicated that the new NHA was only hired on the day of the survey, confirming a period without a licensed administrator in the facility.
