Summary

The facility failed to ensure that grievances were filed and resolved in a timely manner for one resident. The resident, who had diagnoses including major depressive disorder and mild neurocognitive disorder with severely impaired cognition, repeatedly complained to staff about a roommate's television being too loud at night, which interfered with sleep. Despite these complaints being documented in clinical progress notes, there was no evidence that a formal grievance was filed or that the grievance process was followed according to facility policy. Nurse staff acknowledged receiving the complaints and reported speaking to the roommate and notifying the Unit Manager, but did not initiate a grievance form or document a resolution in the grievance log.
