Summary

The facility administration failed to complete annual performance reviews for every nurse aide and did not provide regular in-service education based on the outcomes of such reviews. During the investigation of an allegation of staff-to-resident abuse, it was found that a Geriatric Nursing Assistant (GNA) had only one annual performance evaluation on file since being hired, with no subsequent yearly evaluations available. The Director of Nursing (DON) confirmed that annual performance evaluations for nursing staff had not been conducted since she assumed her position, and the Human Resources director corroborated this information.
