Summary
The facility administration failed to complete annual performance reviews for every nurse aide and did not provide regular in-service education based on the outcomes of such reviews. During the investigation of an allegation of staff-to-resident abuse, it was found that a Geriatric Nursing Assistant (GNA) had only one annual performance evaluation on file since being hired, with no subsequent yearly evaluations available.
