Summary
Facility staff failed to ensure that all nurses and nurse aides received the appropriate training and competencies necessary to care for residents, as evidenced by a review of five employee files. Three staff members were found to be missing documentation of required training in key areas such as behavioral health, tracheotomy care, ventilator care, resident rights, communication, and abuse prevention. Interviews with facility leadership confirmed that these training records could not be located despite searching through employee files.
