The window blinds in the same room were broken and in disrepair. These conditions persisted over several days, as confirmed by repeated observations and interviews with the resident, who stated that the wall and blinds had been in this state for some time and expressed a desire for them to be cleaned and replaced. Additional observations included a large puddle of yellow liquid on the floor in another room, accompanied by a strong urine odor, which was confirmed by a corporate staff member to be unsanitary and in need of cleaning. In a hallway near another room, a ceiling tile was found to have a moderate amount of mold, reportedly due to a leak that had been ongoing for several weeks. Housekeeping and maintenance staff confirmed that the issue had been reported but not resolved, and the ceiling tile required replacement. These findings indicate a failure to maintain a sanitary and orderly environment for residents, staff, and the public.
