Summary

Surveyors observed multiple instances of inadequate housekeeping and maintenance services within the facility, resulting in unsanitary and disordered conditions. In one resident's room, the floor was found to have hair and a dark brown substance near the bed, while the wall near the bed had a moderate amount of a splattered tan and pink substance. The window blinds in the same room were broken and in disrepair. These conditions persisted over several days, as confirmed by repeated observations and interviews with the resident, who stated that the wall and blinds had been in this state for some time and expressed a desire for them to be cleaned and replaced.
