Summary
The facility failed to ensure that a working call light system was available and accessible for residents in their rooms, as required by facility policy. During observations, it was noted that two residents did not have call lights connected to the call light wall system in their rooms. One resident confirmed the absence of a call light and reported having to ambulate to the hallway to seek staff assistance. The other resident also confirmed not having a call light available and was unaware of its location.
