Summary

The facility failed to maintain a safe, clean, and homelike environment for its residents, as evidenced by multiple observations of uncleanliness and physical hazards throughout the building. One resident's fall mat and room floor were found to have sticky food particles and debris, and the mat was described as filthy by a licensed nurse.
