Summary

Surveyors observed that the facility failed to maintain a safe, clean, and homelike environment for its residents. During an initial tour, it was noted that flooring was missing at the entrance of the dining room on the 300 hallway, with an area approximately four feet long by one foot wide covered by water-saturated blankets and surrounded by wet floor caution signs. This condition had persisted for at least two to three weeks, as confirmed by both a resident and a housekeeper.
