Summary
The facility failed to maintain a sanitary, safe, and homelike environment in the main kitchen, as evidenced by dirty kitchen walls and floors. During an initial kitchen tour with the Dietary Food Manager (DFM), surveyors observed that the wall next to the stove top was covered in splashed food and debris, and the floor and baseboard underneath the dishwasher were dirty and had a buildup of debris.
