Surveyors observed multiple environmental deficiencies throughout the facility, including showers in resident bathrooms containing a brown/black furry textured substance, non-functioning water in one shower room, leaking faucets, broken and cracked tiles, exposed unconnected piping, and non-operational ventilation fans in resident rooms.
