Summary

The facility failed to provide the required minimum of 12 hours of in-service education per year for two of three Certified Nursing Assistants (CNAs) reviewed for sufficient and competent staffing. Record reviews showed that both CNAs had been employed at the facility for over 12 months, but their training records did not document completion of the required annual in-service hours for the evaluation period. During staff interviews, facility leadership confirmed that they were unable to produce any records showing that these CNAs had completed the mandated training for the specified period.
