The facility failed to ensure the accuracy of Minimum Data Set (MDS) assessments for residents, as evidenced by the incorrect coding of a resident's PASRR (Preadmission Screening and Resident Review) status. Specifically, a resident with diagnoses including post-traumatic stress disorder, stimulant abuse, and panic disorder was found to have a PASRR Level II documented in the electronic medical record, but the annual MDS assessment incorrectly indicated that the resident did not have a PASRR Level II.
