Summary

The facility failed to provide hospital transfer paperwork for one resident who was discharged to the hospital. Record review showed that the resident, who had a history of stroke and left-side paralysis, was transferred to the hospital, but there was no documentation that current medical information was provided to the receiving hospital at the time of transfer. Staff interview with the DON confirmed that she could not provide documentation that the hospital received the resident's current medical documentation during the transfer.
