Surveyors observed multiple deficiencies in the storage and labeling of drugs and biologicals across three of four medication carts. One medication cart was found unlocked and unattended, with a cup containing two unidentified white medications left on top. Several insulin pens and eye drops were opened without being labeled with the date opened or expiration date, and some insulins that required refrigeration were found in the cart instead of being stored in a refrigerator as per pharmacy instructions. Staff interviews confirmed that medications should be labeled when opened and that insulin should be refrigerated until use, but staff were unable to explain why these procedures were not followed. Additionally, staff acknowledged that the medication cart should have been locked at all times when not in use. Policy reviews indicated that the facility requires all medications and biologicals to be stored in locked compartments and under proper temperature controls, with opened medications labeled with expiration dates. Despite these policies, observations revealed that opened insulins and eye drops were not labeled, and some medications requiring refrigeration were improperly stored in medication carts. The Director of Nursing confirmed expectations for staff to lock carts, check for expired medications daily, and follow pharmacy recommendations for storage and labeling, but these practices were not consistently implemented.
