Summary

Facility staff failed to provide documented evidence of required in-service training for nurse aides. A review of the facility's assessment and training records revealed that only three months of in-service training for Certified Nurse Aides (CNAs) in 2024 were documented, with no staff sign-in sheets attached to the training materials.
