Staff interviews revealed inconsistencies in the communication and posting of activity and outing calendars, with the outings calendar not always being distributed or posted in accessible locations. The activities consultant indicated that quarterly participation assessments should be conducted to adjust activities as needed, but assessments for the residents in question were outdated or did not reflect current preferences. The activities assistant stated that daily activity sheets were distributed and staff attempted to remind residents of activities, but residents reported not being personally invited or encouraged to participate. These findings demonstrate a failure to individualize and update activity programming to meet the socialization and activity needs of the residents.
