The linoleum flooring in these bathrooms was described as old, scratched, and in disrepair, with additional observations of loose dirt debris, black scuff marks, and yellow staining. These conditions were confirmed by both family members and staff, who stated that the bathrooms were not in acceptable or sanitary condition. Facility documentation indicated that housekeeping procedures required daily and thorough cleaning of environmental surfaces, but these standards were not met in the observed bathrooms. Interviews with staff, including the Assistant Director of Nursing and Environmental Services Manager, acknowledged awareness of the maintenance and cleanliness issues in the resident restrooms. Both staff and family members expressed that the bathrooms were not maintained to a homelike or sanitary standard, with one family member stating they would not allow their home restroom to be in such a condition. The facility's own policies required a clean, sanitary environment, but observations and interviews confirmed that these expectations were not being fulfilled in the resident bathrooms at the time of the survey.
