Summary

The facility failed to ensure that abuse prevention training for staff was sufficient, as evidenced by a review of training records and interviews. Specifically, one Certified Nurse Assistant (CNA) had not received abuse prevention training for over a year, with the last documented training occurring more than twelve months prior to the survey.
