Summary

Surveyors observed that the facility failed to maintain one of two laundry service rooms in good repair, as required by policy. The laundry room was found to have multiple areas in disrepair, including cracked and dirty floors, a large hole in the floor, an open pipe in the wall, and a wooden platform supporting chemical buckets that was rotted or water-damaged with holes and cracks. The ceiling had dark spots of unknown origin, peeling and flaking paint, and the door leading to the trash area was broken at the bottom. The floor was also noted to be several different colors, indicating possible staining or further disrepair.
