Failure to Meet Minimum Night Shift Nurse Aide Staffing Requirements
Penalty
Summary
The facility failed to meet the required minimum nurse aide staffing levels on the night shift for one of seven reviewed days. Specifically, on 8/7/25, staffing documents showed that with a census of 94 residents, the facility provided only 36.70 nurse aide hours instead of the required 47.00 hours. This shortfall resulted in not having at least one nurse aide per 15 residents as mandated by regulation. The Nursing Home Administrator confirmed during an interview that the facility did not provide the required number of nurse aides for that shift. No additional details regarding the medical history or condition of individual residents at the time of the deficiency were provided in the report.
Plan Of Correction
The residents had no negative outcome from staffing pattern mentioned in cited survey. The NHA or designee will provide the scheduler with education on Pennsylvania staffing requirements for nurse aide ratios. Daily labor calls and meetings are held to look at the schedule ahead of time to make sure scheduling needs are met. Increase recruitment for new hires will start. Staffing coordinator or designee will audit the ratios weekly for four weeks and monthly for one month. Results of audits will be submitted to the QAPI committee for tracking and further recommendations.
