Summary

The facility failed to meet the required minimum nurse aide staffing levels on the night shift for one of seven reviewed days. Specifically, on 8/7/25, staffing documents showed that with a census of 94 residents, the facility provided only 36.70 nurse aide hours instead of the required 47.00 hours.
