Summary

Kadima Rehabilitation & Nursing at Lititz was found to be non-compliant with federal emergency preparedness requirements during a Medicare/Medicaid Recertification Survey. Specifically, the facility failed to provide an updated all-hazards risk assessment as required by regulation.
