Summary

The facility failed to provide both group and individual activities for residents in the memory care unit, as required by their comprehensive assessments and care plans. Observations revealed that residents spent extended periods sitting in the day room with only a television or radio for stimulation, and there was no activity calendar or evidence of activity materials such as puzzles, books, or art supplies available in the unit.
