Summary

The facility failed to effectively utilize its Quality Assessment and Performance Improvement (QAPI) program to address an ongoing issue with missing covers on all three dumpsters. Review of the Food & Nutrition Services: Daily Supervisor Rounds Checklist showed that the task 'Trash Lids closed and clean' was not checked off on any day between March 2, 2025, and April 17, 2025. During an interview and record review, the Administrator confirmed that replacing the dumpster covers was a current QAPI project, but the bins remained uncovered due to repeated cancellations by the waste management company.
