A review of the facility's recipe and policies confirmed that the pureed bread should have a smooth, lump-free, and extremely thick consistency, similar to mashed potatoes or pudding. The Registered Dietician stated that the cook did not follow the recipe, which led to the improper consistency of the pureed bread.
