Summary
A resident with a diagnosis of Post-Traumatic Stress Disorder (PTSD), identified as having an onset date of 08/26/24, did not have this condition addressed in their care plan. Upon review of the resident's records, there was no evidence that PTSD was included or considered in the care planning process. Additionally, during an interview, the social worker assigned to the resident was unaware of the PTSD diagnosis and confirmed that the resident was not receiving trauma-informed care.
