Summary
The facility failed to ensure that a resident's Pre-Admission Screening (PAS) accurately reflected the resident's pre-admission diagnosis. During a record review, it was found that the PAS submitted for the resident did not indicate any diagnoses in Sections III (MI/MR Assessment) and V (Supplemental Questions for Major Mental Illness or suspected MI), despite the resident having an admission diagnosis of Bipolar Disorder, Unspecified. The Director of Nursing confirmed that the bipolar diagnosis was not included on the initial PAS and acknowledged this as an oversight.
