Summary

A deficiency was identified when a resident, who was cognitively intact and able to communicate their needs, was found to have a call light/TV cord in their room with exposed colored wires near the control. The resident reported having informed staff about the damaged cord and requested a replacement, but no action was taken to address the issue.
