Summary

The facility failed to ensure that hospice documentation was complete for one resident. The resident, who had multiple diagnoses including senile degeneration of the brain, major depressive disorder, generalized anxiety disorder, unspecified psychosis, hypertension, glaucoma, vitamin B deficiency, muscle weakness, and gait abnormalities, was readmitted to the facility and placed on hospice care. Upon review, it was found that the most recent hospice plan of care and recertification period had expired, and there was incomplete or missing correspondence from the hospice staff providing care.
