Summary

The facility failed to provide required annual compliance and ethics training to four out of five reviewed staff members, including three nurse aides and one LPN. According to the facility's own assessment and in-service training policy, all personnel are mandated to participate in regular education sessions covering topics such as compliance and ethics.
