Summary
A resident admitted with a diagnosis of depression was prescribed duloxetine and Wellbutrin, both antidepressant medications, as indicated in the physician orders and medication administration record. Despite receiving these medications daily, there was no documentation in the medical record that the resident had been informed in advance about the risks and benefits associated with either medication.
