The facility failed to provide scheduled bathing assistance to two residents who were dependent on staff for activities of daily living (ADL), specifically bathing. This resident was scheduled for twice-weekly bathing, but records indicated missed scheduled baths and a lack of documentation for several weeks. While some refusals were documented in the past, there was no evidence that the resident was offered or received bathing assistance during the period in question, nor that refusals were documented for the missed dates. Staff interviews confirmed the expectation to follow the bathing schedule and document all care and refusals, but acknowledged the absence of required documentation for this resident as well. Facility policy required that all residents be bathed at least once a week and according to a set schedule, with all bathing activities and refusals documented. The failure to provide and document scheduled bathing services for these two residents, both of whom were dependent on staff for personal care, constituted a deficiency in meeting the facility's obligations for ADL assistance and hygiene maintenance.
