Summary

The facility failed to develop and implement comprehensive care plans for two residents with specific clinical needs. One resident, who was cognitively intact and had a diagnosis of paroxysmal atrial fibrillation, was prescribed Apixaban, an anticoagulant, for peripheral vascular disease.
