Summary
The facility failed to ensure the accuracy of the Minimum Data Set (MDS) assessment for one resident regarding anticoagulant medication use. Record review showed that the resident was prescribed aspirin and clopidogrel, but not an anticoagulant. However, the most recent MDS assessment incorrectly documented that the resident had received an anticoagulant in the past 14 days. During an interview, the Assistant Director of Nursing/MDS Coordinator admitted to mistakenly believing the resident was on an anticoagulant, which led to the inaccurate MDS entry. This inaccuracy was confirmed by both the medication orders and the staff interview.
