Summary

The facility failed to report two incidents of elopement involving two residents to the State Agency. On the specified date, a maintenance assistant observed the two residents in the facility's south parking lot area, and one of the residents later confirmed that both had exited through the back gate and were subsequently brought back inside by staff. During an interview, the Administrator acknowledged awareness that the residents were found outside by the dumpsters but stated that the incident was not considered an elopement because the residents remained on facility grounds.
