Summary

The facility failed to provide a quarterly financial statement to a resident who had requested to receive statements for her personal trust account. The resident, who was identified as cognitively intact on her annual Minimum Data Set (MDS), reported that she was not receiving statements and was unsure of her account balance. Interviews with the business office manager and the administrator confirmed that the statements were being sent to the resident's former home address instead of being provided directly to her as required.
