Summary

The facility failed to maintain a safe, functional, sanitary, and comfortable environment for residents, staff, and the public. Observations revealed multiple environmental deficiencies, including detached and soiled hallway ceiling tiles with visible discoloration, frayed edges, and signs of repeated water damage.
