Summary

Surveyors observed that the facility failed to maintain safe and sanitary food storage practices for all residents. Specifically, the inside gasket of the kitchen ice machine was found to have yellow and white buildup, which the Dietary Supervisor acknowledged as water residue that should not be present and could cause contamination.
