Summary

The facility failed to maintain a clean, homelike, and visually appealing environment in the rooms of two residents, as evidenced by large areas of discoloration and peeling paint on the walls near the heads of their beds. These deficiencies were observed during initial tours and confirmed by both nursing and maintenance staff, who acknowledged the poor condition of the walls and agreed that repairs were needed. The issues were visible to residents and their visitors, with one resident's family member regularly sitting in a position where the damaged wall was in clear view.
