Summary

The facility failed to conduct and document annual performance reviews for 40 out of 69 Certified Nursing Assistants (CNAs) who had been employed for over one year, as required by federal regulation. During a review of employee files, it was found that no annual performance reviews were documented for multiple CNAs, including those hired as far back as 2017 and 2021.
