Summary

The facility failed to ensure that its activities program was directed by a qualified professional, as required. Review of the personnel file for the Activity Director revealed that the individual did not possess an Activity Director Certification and had only two years of experience in a social or recreational program. The Activity Director had been hired in January and previously worked as a CNA and in transportation, with no prior experience as an activity director. She reported having completed only one training class and was not yet certified, although she was enrolled in a certification course scheduled to begin in several months. During this period, she also performed duties as a restorative aide and occasionally drove the facility van.
